DECLARATIONS AND GUARANTEES
- I have answered all the questions regarding my health and skin correctly and to the best of my knowledge, I am aware that some ailments 
and medication can affect healing of the skin and response to skin treatments. It is my responsibility to ensure that I inform KOJA Skin & 
Laser Experts if I begin a course of medication during my course of treatment.
- I hereby declare that I fully understand all the information provided (including, but not limited to the Product, the Procedure, the Scope, 
Risks, Side Effects and Contraindications) provided herein, as well as verbally by the practitioner/ supervising practitioner/trainee/medical 
assistant), that I have provided the practitioner only with true and real information and I expressly and informed consent to undergo the 
Procedure.
- I hereby expressly declare the following:
• the procedure, as well as the information provided herein, have been explained to me in a language and terminology that I 
completely understand, / am able to take an informed decision and I understand the Procedure to which / consent.
• I have been given sufficient time to consider and evaluate the procedure that I intend to undergo, to ask additional questions to 
other persons, to evaluate all the information, including risks and complications so that I consent to the procedure in a free and informed 
manner.
• I understand and expressly accept the risks, consequences, complications and side effects of the procedure, which may occur, 
even the unforeseen ones and I hereby declare I will not hold the practitioner performing the procedure liable for any of them.
• I have been informed regarding the period of healing, forbidden or permitted activities, all before and after procedure rules, 
including the ones that directly affect the area on which the procedure was performed.
• I have been informed on the information given by the producer/manufacturer of the Product that shall be used in performing the 
procedure, and that the practitioner does not take any responsibility for the product, or it's inefficiency.
• I have been informed and I understand that the practitioner does not take any responsibility for the quality and the efficiency of 
the result of this procedure, only for the diligence with which every practitioner should perform such procedures.
• I have been informed regarding all the activities that may increase the risks of complications (including smoking, alcohol, etc), 
as well as medical treatments that may interfere with the performance of the procedure, in accordance with the existing information herein 
and also in the prospect/product details provided by the manufacturer.
• I have disclosed my state of health without reserve, to allow the practitioner to evaluate any counter-indications;
• I give my consent for KOJA SKIN & LASER EXPERTS the practitioner/the supervising practitioner or any legal entity in 
connection with the practitioner, to acquire my personal and clinical information.
• I am not taking antibiotics, amino-glycosides, spectinomycin or other medicines that interfere with neuromuscular transmission;
• I am not pregnant (even hypothetical or presumed) or breastfeeding.
• I do not fall under any of the cases/categories that are contraindicated for this procedure.
- Agree to the course of treatment(s) and product(s) described on this form and that the cost and payment method has been discussed and 
agreed with me.
- l understand that these terms form the basis of a contract between myself and KOJA Skin & Laser Experts.
- I understand the importance of intervals advised to me during my prescribed plan of skin treatments.
- I have been advised on the benefit and potential adverse reactions and have also been given aftercare advice for my prop skin treatment.
- I have also been informed that the results differ and success of treatment(s) varies from person to person. KOJA Skin & Laser Experts 
can offer no guarantee or warranty on effectiveness of any treatments.
- I have also been informed products prescribed to me may contain active, results driven ingredients and it is
likely my skin show changes within the first few weeks of use.
- l agree to follow the aftercare advice given to me. I understand that failure to follow the advice may increase the chance of scarring or 
skin textural changes. This has been discussed with me.
- I agree to inform KOJA Skin & Laser Experts of any changes in medication or in my general health which may affect my treatment.
- l understand that should I agree to the course of treatment described, when I am currently taking, or have recently taken, pharmaceutical 
product or other medication or drug with reported photosensitive properties, then there is an increased risk of rash occurring in the 
treatment areas.
- I must immediately inform the Clinic and practitioner if there are any changes to general health, possible pregnancy or change of 
medication.
- I understand if I fall pregnant during my treatment course, I will have to discontinue my treatment and re-commence post-pregnancy

• I understand that there are other treatment options available. I also understand that adjustments to my treatment
  programme may be necessary (including the possibility of having to discontinue my treatment programme) if I experience unusual or 
  adverse skin reactions.
  - l understand that no warranty or guarantee has been made to me as to result or cure. It is possible that results might not come up to 
  expectations or goals. KOJA Skin & Laser Experts is not to be held financially liable provided treatments have been carried out in 
  accordance with treatment guidelines and protocols.
  - I understand that the information I have given is confidential and protected under the data Protection Act and will not be released to a 3rd 
  Party same as permitted by law.
  - The information provided is honest and truthful.
  OTHER OBLIGATIONS & CONFIDENTIALITY
  I also expressly declare and undertake the following:
  (a) I undertake the obligation to keep confidential any information that I have/ obtain/that is brought to my knowledge in 
  connection with the procedure, before, during or after the procedure.
  (b) I undertake the obligation not the use, in any way, direct or indirect, virtual, mass or social media the name or any information 
  related to the Practitioner, the supervising practitioner, the staff of the practitioner/clinic, the clinic, the physical or legal entities involved/ 
  or representatives of such, the procedure, the persons participating at the procedure, or any other information from point (a) above, 
  without the written and express consent of the entities involved.
  (c) in case any situation, dispute or discontent of any nature may arise regarding the Procedure or the results/side 
  effects/consequences of such, I undertake the obligation to notify the parties involved and to put the best effort to solve it amicably and not 
  to use in any way, direct or indirect, nor to disseminate to anyone information regarding such situation. If the situation is not solved 
  amicably, the case can be submitted before the competent court, but all the obligations undertaken herein remain in force.
  (d) I undertake the obligation not to act in any way that might damage in any manner the practitioner supervising practitioner, the 
  clinic, the staff or representatives, their image or anything and anyone related to them. In case I shall not observe/I shall breach any of the 
  obligations stated herein, I hereby undertake to pay the damaged party the sum of 50.000,00 (fifty thousand gbp) GBP as damages, such 
  remedy being in addition to any other remedies provided by law.
   
   Signature of the Patient that expressly and informed consents to undergo the medical procedure.
  REFUND POLICY
  KOJA Skin & Laser Experts do not offer refunds for treatment received or products used, however if you change your mind within 14 days
  of purchase if no sessions have been redeemed you are entitled to a refund. If for any medical reason you are advised not to continue with 
  your course of treatments, please obtain a medical report from your UK registered doctor. If you have used any sessions, then this will be 
  deducted from your refund value as a pay as you go cost. I understand that KOJA Skin & Laser Experts can’t be held responsible for any 
  unknown side effects which may occur as a result of having laser treatments. If you wish to amend your package KOJA Skin & Laser 
  Experts can assist with this.
  COVID update on refunds
  Any refund requests will need to be notified to the clinic within the first 14 days of purchasing.
  Refunds are only able to be issued on the basis of a medical condition with a doctor's document provided - we do not class pregnancy as a 
  medical condition. We will only be able to issue a refund on long term medical conditions, as all our packages do not have an expiry date 
  so you are able to use this at any time in the future. Please contact your assigned clinic for a refund request - your refund will be 
  acknowledged within 72 hours and will only be issued once we have receipt of the Medical document once this has been confirmed by a 
  member of our team. KOJA Skin & Laser Experts will do our utmost to assist and help you within your journey going forward.
  LATENESS POLICY
  If arrival is delayed, we will make every effort to accommodate your full appointment but this is not always possible. Service team may be 
  abbreviated or cancelled to avoid delays to other clients. Last minute changes to appointments will be honoured only if service 
  appointment is available for a later time in the day (of your appointment). Please be aware that with certain skin treatments it may be 
  necessary for your comfort to arrive 30 minutes prior to your treatment (this will have been advised to you) for the application of a 
  numbing cream. Please be advised any lateness will forfeit a skin treatment session if the treatment cannot be carried out or treated in the 
  time available.
  CANCELLATION POLICY
  A session will be redeemed or a charge will be applicable for clients who do not provide 48 hours notice. Depending on whether you are on
  a course of treatments or purchasing a single session:
  - If you give at least 48 hours notice of cancellation no penalty will be incurred 
• - If you cancel less than 48 hours before your appointment or do not show up for your booked appointment, then you will lose that session
  MEDICAL CONDITIONS
  Please notify us before reserving your treatments if you have high blood pressure, special physical concerns, diabetes, or are pregnant or 
  have a history of skin pigmentation.
  PAYMENT POLICY
  Correct payment must be received before treatments start on each scheduled booked appointment. This is generally in taken in the clinic or
  over the phone. If your payment is not received, we have the right to refuse to treat you. If a payment plan such as instalments has been 
  agreed, Instalments must be paid at correct intervals as agreed by Clinic and customers.
  PRACTITIONERS
  While we will make every effort to meet your requests, we cannot guarantee a specific practitioner or a practitioner of a specific gender. 
  All practitioners are trained in all undertaken treatments and protocols to ensure your comfort and treatment efficacy all times.
  MAKE UP
  Please be advised makeup, deodorant, perfume and oil based products must be removed before treatment. Practitioners will cleanse the 
  area however we cannot be held responsible for any visible products that may lay dormant in the skin pores and will cause an adverse 
  reaction.
  LIABILITY POLICY
  KOJA Skin & Laser Experts not be liable in contract, tort or otherwise for any economic loss (including, without limitation, loss of profit), 
  or for any other special, indirect or consequential loss or damage arising out of, or in connection with, its provision of a goods and/or 
  services to the client. Your statutory will be protected. It is the client's responsibility to ensure that he/she provided us with all relevant 
  medical details prior to each treatment. We will not be liable for any damage that occurs as a result of the client's failure to disclose such 
  details.
  The client agrees to comply with all instructions and/or recommendations given to them by, or on behalf of, KOJA Skin & Laser Experts 
  regarding the care of a treated area.
  CONSENT TO PHOTOGRAPHY & VIDEOGRAPHY
  Your practitioner treating you requires photographs & videos to help monitor and record your treatment process. Without the photographs 
  and videos it will be difficult to confirm the success of the treatment and determine accurately your condition at the start of your treatment 
  plan. All photographs and videos taken will be confidential and will be kept in your personal file along with other confidential records and 
  notes relating to your treatment.
  All photography & videos will be taken in the most suitable angles and position to showcase the treatment outcome in the best form. 
  Subsequent photographs ans videos taken as the course progresses will be of the same area and taken with the same magnification. 
  Photographs will not directly identify you, instead a code or unique identification will be attached to each photograph so that the 
  photograph will only be identifiable to restricted KOJA Skin & Laser Experts staff.
  Photographs will be held for a minimum of 7 years. You are entitled, at any time to change your mind regarding consent, even upon the 
  signing of the consent form.
  KOJA Skin & Laser Experts effectively manage treatment outcomes without the consent and use
  photographic evidence.
